REVÍ is intended for specific, non-emergency hair restoration and wellness-related concerns. If you believe you are experiencing a medical emergency, call 911 or seek immediate emergency medical care.

These Terms & Conditions govern your access to and use of the REVÍ website, intake forms, patient portal, communications, content, features, products, and related services collectively referred to as the “Platform” or “Services.” The terms “REVÍ,” “we,” “us,” and “our” refer to REVÍ Health and its related brands, service providers, contractors, partners, affiliates, successors, and assigns. The terms “you” and “your” refer to the individual accessing or using the Platform.

By accessing or using the Platform, creating an account, submitting an intake, purchasing a product, communicating with support, or requesting telehealth services, you acknowledge that you have read, understood, and agree to be bound by these Terms and any policies referenced herein, including our Privacy Policy, Notice of Privacy Practices, Cancellation & Refund Policy, Medical Consent, and Patient Bill of Rights.

THESE TERMS CONTAIN IMPORTANT PROVISIONS REGARDING TELEHEALTH SERVICES, PAYMENT, LIMITATIONS OF LIABILITY, DISCLAIMERS, ARBITRATION, CLASS ACTION WAIVER, AND JURY TRIAL WAIVER. PLEASE READ THEM CAREFULLY.

I. Introduction

REVÍ operates a digital platform designed to support individuals seeking information, consultation, and treatment options related to hair loss, hair thinning, scalp wellness, and hair restoration. REVÍ does not provide emergency medical services. REVÍ does not replace your primary care provider, dermatologist, or other healthcare professional.

Certain services made available through the Platform may be provided by independent licensed healthcare professionals, professional medical groups, pharmacies, laboratories, fulfillment partners, payment processors, technology vendors, and other third-party service providers. REVÍ may provide administrative, technology, marketing, care coordination, and customer support services in connection with those services.

II. Modification of Terms

We may update or revise these Terms at any time. When we do, we may update the “Last updated” date above. Your continued use of the Platform after revised Terms are posted means you accept the revised Terms. If you do not agree with the revised Terms, you should stop using the Platform.

III. Description of REVÍ Services

The Platform may facilitate one or more of the following:

You understand that the Platform collects information from you so that an independent licensed provider may determine, in their professional judgment, whether a treatment, medication, device, supplement, additional testing, or referral is appropriate. Payment or completion of an intake does not guarantee a prescription, diagnosis, treatment approval, product shipment, or medical outcome.

IV. Independent Medical Providers

REVÍ is not a medical practice and does not itself practice medicine, diagnose medical conditions, prescribe medications, or provide clinical treatment. Medical services, when available, are provided by independent licensed healthcare providers or professional entities. Those providers are solely responsible for the clinical services they provide, including diagnosis, treatment decisions, prescriptions, medical advice, and follow-up recommendations.

You understand and agree that REVÍ does not control or interfere with a provider’s independent professional judgment. If a provider determines that treatment is not appropriate, REVÍ is not responsible for that decision.

V. Pharmacy, Fulfillment, and Product Partners

REVÍ is not a pharmacy and does not dispense prescription medication. Prescription medications, when prescribed, may be fulfilled by licensed pharmacies or fulfillment partners. Non-prescription products may be fulfilled by REVÍ or third-party vendors.

You understand that pharmacies and fulfillment partners may require additional information, may decline to fill a prescription, may substitute packaging or formulation where permitted, and may operate under their own professional and regulatory obligations.

VI. Eligibility

To use the Platform, you represent that:

VII. Availability of Services

Services may not be available in all states, territories, or countries. Availability may vary based on provider licensing, pharmacy restrictions, state laws, product availability, shipping limitations, and clinical eligibility requirements. REVÍ may change, suspend, or discontinue any service or product at any time.

VIII. Account Registration and Security

You may be required to create an account or provide identifying information to access certain services. You are responsible for maintaining the confidentiality of your login credentials and for all activity occurring under your account.

You agree to notify REVÍ promptly if you suspect unauthorized access, misuse of your account, or any security issue. REVÍ may suspend or terminate account access if we believe your account has been compromised or used in violation of these Terms.

IX. Your Responsibilities

You agree that:

Incomplete or inaccurate information may affect your eligibility, safety, treatment plan, or results. REVÍ and the independent providers rely on the information you submit.

X. Hair Restoration Results and Medical Limitations

Hair restoration outcomes vary. Individual results may depend on age, genetics, sex, hormone profile, medical history, hair loss stage, medication adherence, scalp condition, nutrition, stress, postpartum status, medications, and other factors.

REVÍ does not guarantee that any product, prescription, supplement, serum, spray, peptide treatment, microneedling tool, or hair restoration plan will produce regrowth, stop shedding, restore a hairline, reverse baldness, or produce a particular cosmetic result.

Information on the Platform is for informational and educational purposes and is not a substitute for professional medical advice, diagnosis, or treatment.

XI. Telehealth Consent

Telehealth involves the use of electronic communications, online questionnaires, secure messaging, video, audio, phone, photos, remote monitoring, or other technology to allow healthcare providers to evaluate and communicate with patients remotely.

Telehealth may offer convenience and access, but it also has limitations. Providers may not be able to perform a physical examination, may request additional information or photos, may recommend in-person evaluation, and may decline to treat if telehealth is not appropriate.

By using REVÍ services, you consent to receive healthcare services through telehealth where permitted and agree to review and accept REVÍ’s Medical Consent before receiving applicable services.

XII. Prescription Products and Treatment Safety

Some hair restoration treatments may involve prescription medications such as minoxidil, finasteride, compounded topical treatments, or other provider-recommended therapies. Prescription treatment is only available if an independent licensed provider determines it is appropriate.

You agree to read all product labels, medication guides, provider instructions, contraindication information, and pharmacy materials. You agree to contact a healthcare professional if you experience side effects, allergic reactions, unexpected symptoms, worsening scalp irritation, dizziness, chest pain, swelling, sexual side effects, mood changes, pregnancy-related concerns, or any other concerning symptom.

XIII. Payment, Billing, and Fees

By submitting payment information, you authorize REVÍ, its affiliates, or payment processors to charge your selected payment method for applicable fees, including consultation fees, product charges, subscription fees, shipping charges, taxes, and other amounts disclosed at checkout.

Fees may include amounts payable to independent medical providers, pharmacies, fulfillment partners, or other service providers. Final charges may vary depending on the treatment prescribed, product selection, pharmacy fulfillment, state requirements, shipping method, or plan chosen.

Unless otherwise stated, healthcare consultation fees may be non-refundable once a provider review has occurred. Prescription products generally cannot be returned for reuse or resale. Please review our Cancellation & Refund Policy for additional details.

XIV. Subscriptions, Refills, and Auto-Renewal

Some products or plans may be offered on a subscription or recurring refill basis. By enrolling in a subscription, you authorize recurring charges until you cancel, pause, or modify your plan according to the instructions provided in your account or our policies.

You are responsible for reviewing refill dates, updating payment information, updating shipping information, and notifying us of any changes to your health or treatment eligibility before refills are processed.

XV. Insurance

REVÍ services are typically cash-pay and may not be covered by insurance, Medicare, Medicaid, or other government healthcare programs. You are responsible for all fees unless otherwise stated. REVÍ does not guarantee reimbursement from any insurer, HSA, FSA, employer plan, or third-party payer.

XVI. Privacy

REVÍ respects the confidentiality of your information. Please review our Privacy Policy and Notice of Privacy Practices to understand how information may be collected, used, disclosed, stored, and protected.

XVII. Communications Consent

By using the Platform, you consent to receive communications from REVÍ, independent providers, pharmacies, and service partners by email, phone, SMS, portal message, push notification, or other electronic means. These communications may include intake reminders, treatment updates, refill notices, account notices, support messages, marketing messages where permitted, and legal disclosures.

You understand that standard messaging and data rates may apply. You may opt out of certain marketing communications, but transactional or healthcare-related communications may still be sent where necessary.

XIX. Intellectual Property

The Platform, including text, graphics, images, logos, design, user interface, software, videos, icons, trademarks, trade names, and other materials, is owned by REVÍ or its licensors and is protected by intellectual property laws.

REVÍ grants you a limited, revocable, non-exclusive, non-transferable license to access and use the Platform for personal, non-commercial purposes. No rights are granted except as expressly stated in these Terms.

XX. User Content and Submitted Information

You may submit information, photos, messages, survey responses, reviews, or other content to REVÍ. You grant REVÍ and its service providers permission to use, store, process, transmit, and display such content as reasonably necessary to provide services, operate the Platform, comply with law, improve services, and communicate with you.

You are responsible for ensuring that anything you submit is accurate and that you have the right to submit it.

XXI. Third-Party Websites and Services

The Platform may contain links or references to third-party websites, services, pharmacies, payment processors, laboratories, shipping carriers, social media platforms, or educational resources. REVÍ does not control those third parties and is not responsible for their content, practices, products, or services.

XXII. Disclaimer of Warranties

THE PLATFORM AND SERVICES ARE PROVIDED ON AN “AS IS” AND “AS AVAILABLE” BASIS. TO THE FULLEST EXTENT PERMITTED BY LAW, REVÍ DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE, NON-INFRINGEMENT, SECURITY, ACCURACY, RELIABILITY, AVAILABILITY, AND ERROR-FREE OPERATION.

REVÍ DOES NOT WARRANT THAT THE PLATFORM WILL BE UNINTERRUPTED, SECURE, FREE OF VIRUSES, ERROR-FREE, OR THAT ANY DEFECT WILL BE CORRECTED.

XXIII. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY LAW, REVÍ AND ITS RELATED PARTIES SHALL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES, INCLUDING LOST PROFITS, LOSS OF DATA, LOSS OF GOODWILL, SERVICE INTERRUPTION, COMPUTER DAMAGE, PERSONAL INJURY, EMOTIONAL DISTRESS, OR OTHER DAMAGES ARISING FROM OR RELATED TO YOUR USE OF THE PLATFORM OR SERVICES.

TO THE EXTENT PERMITTED BY LAW, REVÍ’S TOTAL LIABILITY FOR CLAIMS ARISING OUT OF OR RELATED TO THESE TERMS SHALL NOT EXCEED THE GREATER OF THE AMOUNT YOU PAID TO REVÍ IN THE THREE MONTHS BEFORE THE CLAIM OR ONE HUNDRED DOLLARS ($100).

XXIV. Indemnification

You agree to indemnify, defend, and hold harmless REVÍ, its affiliates, officers, directors, employees, contractors, service providers, licensors, successors, and assigns from and against claims, liabilities, damages, losses, costs, and expenses, including reasonable attorneys’ fees, arising out of or related to your violation of these Terms, misuse of the Platform, violation of law, or submission of inaccurate information.

XXV. Suspension and Termination

REVÍ may suspend, restrict, or terminate your access to the Platform at any time, with or without notice, if we believe you violated these Terms, created risk, provided inaccurate information, misused the Platform, failed to pay amounts owed, or if access is no longer appropriate or available.

Certain provisions will survive termination, including provisions related to payment, intellectual property, privacy, disclaimers, limitation of liability, indemnification, dispute resolution, and miscellaneous terms.

XXVI. Governing Law

These Terms and any dispute arising from or related to these Terms or the Platform shall be governed by the laws of the state where REVÍ is organized, without regard to conflict-of-law principles, unless applicable law requires otherwise.

XXVII. Informal Dispute Resolution

Before initiating arbitration or legal proceedings, you and REVÍ agree to attempt to resolve disputes informally. You may send a written notice describing the dispute to support@revihealth.com. The parties will attempt in good faith to resolve the dispute within sixty (60) days.

XXVIII. Arbitration Agreement and Class Action Waiver

Except for claims that may be brought in small claims court or claims involving intellectual property rights, you and REVÍ agree that disputes arising out of or relating to these Terms, the Platform, your account, purchases, communications, products, or services will be resolved by binding individual arbitration rather than in court.

You and REVÍ waive the right to a jury trial and waive the right to participate in a class action, class arbitration, collective action, representative action, private attorney general action, or consolidated proceeding, except where prohibited by law.

You may opt out of this arbitration agreement by sending written notice to support@revihealth.com within thirty (30) days of first accepting these Terms. Your opt-out notice must include your name, email address associated with your account, mailing address, and a clear statement that you wish to opt out of arbitration.

XXX. Miscellaneous

These Terms, together with referenced policies, constitute the entire agreement between you and REVÍ regarding the Platform and Services. If any provision is found invalid or unenforceable, the remaining provisions will continue in effect. REVÍ’s failure to enforce any provision is not a waiver of its right to do so later. You may not assign these Terms without REVÍ’s prior written consent.
